In short, yes. Powders for extemporaneous compounding sit in a unique therapeutic category with its own set of rules. When you choose to use a compounding powder, the expectation is that you will be combining this with other therapeutic nutrients and ingredients to create a personalised medicine. Single ingredient prescribing is, however, possible with extemporaneous compounding ingredients with best practice to ensure that the amount of product in the jar represents the full course for the patient. The quantity of any ingredient you use will be calculated in relation to the prescription length and daily required dose, which is completely specific to an individual patient’s presentation and needs.

  • Nitroglycerin: NAC can increase the risk of hypotension and headaches when taken with intravenous or transdermal nitroglycerin.

NAC has been studied in pregnant women for specific applications including preeclampsia and threatened preterm labour. When used orally, N-acetyl cysteine crosses the placenta, but has not been associated with adverse effects to the foetus. N-acetyl cysteine should only be used in pregnancy when clearly indicated.

  • As NAC can be very strong on the stomach, general recommendations are to take it with a small amount of food. In certain instances taking on an empty stomach is absolutely applicable. Check with the literature relating to the indication.
  • The common range for NAC is 1,000 mg – 3,000 mg per day. It is generally well tolerated at these levels, but monitor.
